Our hospital discharge service offers practical help, support, advice, problem solving and signposting when you, or the person you care for, has had a spell in hospital.

Following registration with the service you will be allocated a dedicated Hospital Discharge worker who will contact you following discharge, then again after 2 weeks, 4 weeks and 6 weeks.

If at any time during this period you are finding things difficult the Hospital Discharge worker can provide more regular support, advice, guidance and advocacy, as well as access to Devon Carers services.

If needed the Hospital Discharge worker can also arrange for tailor made packages of practical help and support including:


  • help with meals/cooking/cleaning/shopping
  • collecting prescriptions
  • transport
